Quick Reference Sheet for Crime Victims Compensation Form


An Application must be filed by, or on behalf of a person who was injured or died as a result of the crime. The Crime Victims Compensation Program ("the Program") may help with expenses and losses directly related to the crime.

ELIGIBILITY REQUIREMENTS (some exceptions may apply)

  • The crime must have been reported to law enforcement within 72 hours of its occurrence. For sex-based crimes, the notification must have made within 7 days of the perpetration or discovery of the crime.
  • The application must be filed within 2 years of the crime date or discovery of the crime.
  • The Program can only assist victims who have suffered from a compensable crime as defined under the Crime Victims Compensation Act. 740 ILCS 45/.
  • The crime must have occurred in Illinois or in a country without a crime victims compensation program.
  • The victim must cooperate with the police investigation and prosecution of the crime.
  • The victim's acts or conduct must not have provoked or substantially contributed to the injury.

How to file for the Crime Victims Compensation Program

  • Fill out an application. (Separate applications must be filed for each injured victim.)
  • Attach itemized copies of medical, hospital and counseling bills.
  • All of the bills must be submitted to all other sources of recovery available to the victim. This includes, but is not limited to, any medical insurance, including Medicaid, Medicare, life insurance, and workers' compensation.
  • The Program may pay incurred compensable expenses up to $27,000.00.
  • Submit the completed application.
  • The applicant will receive a letter acknowledging receipt of the application.
  • The Attorney General will investigate your claim and file a report with the Court of Claims.
